Outcome-Driven Care | KPIs & Measurement

Explaining Our KPIs, Data Capture, and Impact Reporting

As commissioning practice becomes more outcome-led, care providers must demonstrate more than compliance — we must demonstrate progress.

At Nwando Care, we measure impact in ways that matter: safety, emotional wellbeing, placement stability, engagement with education, and relationship building. These are the cornerstones of our internal outcome framework, and they are recorded, analysed and shared in a way that aligns with local authority reporting expectations.

We track several key indicators:

Placement stability after 3 and 6 months

Educational attendance and engagement (PEP aligned)

Reduction in critical incidents (within first 8 weeks)

Emotional wellbeing (based on keyworker reports and child self-assessment)

Progress against individual care plan goals

Each child’s progress is reviewed monthly by our management team, and every review includes a combination of narrative, data, and professional judgement. These reports are shared with the placing authority and included in LAC reviews.

Importantly, our outcomes aren’t just recorded — they’re discussed. We use key indicators as part of reflective supervision with staff, in multi-agency planning meetings, and to inform continuous improvement across our homes. Data informs how we recruit, how we allocate staff, how we update risk assessments, and how we shape behaviour support plans.

We don’t claim perfection. But we do offer transparency. Commissioners are welcome to request anonymised quarterly summaries, case studies, or home-specific outcomes dashboards. These are especially useful during framework reviews, contract monitoring, or inspection preparation.

We also welcome joint learning — for example, reviewing placement success factors after a transition, or sharing anonymised data across partnerships to improve practice.

Our goal is simple: to show not only what we do — but what it achieves.
