Our referral process is straightforward, responsive, and GDPR-compliant. Whether you're referring for domiciliary care, supported living, reablement, or a children’s residential home, our team will guide you through the next steps.

Who Can Refer?

We accept professional referrals from:

NHS discharge and community health teams

Local authority social workers and brokerage

CHC / S117 care coordinators

SEND officers and education teams

CAMHS and youth justice professionals

Approved advocates and legal representatives

Please note: We do not currently accept private residential referrals for children unless through a commissioning body.

What We Need from You

To process your referral, we’ll ask for:

Basic client/child information (name, age, presenting needs)

Current health, care and risk history

Any education, behavioural or housing plans

Funding source and placement objectives

Safeguarding or communication requirements

Referrer details and preferred contact method

All information is handled securely and in line with UK GDPR and safeguarding protocols.

Adult Referrals

We accept referrals for:

Domiciliary and live-in care

Supported living (including forensic and high-risk needs)

Reablement or hospital discharge (D2A)

Complex, dual-diagnosis and high-need placements

End-of-life or palliative care

Children’s Referrals

We accept referrals for:

Children aged 8–17 with emotional, behavioural or neurodevelopmental needs

Young people with autism, ADHD, trauma or EHCP requirements

Placement breakdown or emergency housing needs

All placements are in Ofsted-registered homes with 24/7 trauma-informed care.
